2025 Outstanding Student Leader Award & Excellence in Student Activities Award Application
Thank you for your interest in applying for these important awards. These are competitive awards, as up to five Outstanding Student Leader Awards and up to 10 Excellence in Student Activities Awards will be granted. Therefore, please fill in the following information carefully and completely. It will be reviewed by a committee of faculty who will make the final selections.

Instructions for completing this application
- Complete all sections.
- Be sure that all words are spelled correctly and that your sentences and paragraphs are grammatically correct.
- You are asked to list up to three (3) faculty members as references. Faculty members should know you from the club or athletics that you are involved in, and they should know you well enough to vouch for your service and accomplishments.
- We suggest that you reach out to each faculty member you have listed and inform them that they may be receiving an email from the Student Awards Subcommittee to request their recommendation. And, of course, remember to thank them!

Applicant Criteria
Outstanding Student Leader Award:
- Full-time student or continuing part-time student eight (8) credits or the equivalent) per semester
- 3.5 cumulative GPA
- 42 Credit hours (or the equivalent) completed with no more than 20 transfer credits
- No more than one “W” grade
- Holds an Executive Board or a Team Captain/Leader position and has made significant contribution to student activities or athletics in this position
Excellence in Student Activities Award:
- At least two semesters in attendance (includes the current semester in which the student applies)
- 3.0 cumulative GPA
- Must complete an average of eight (8) credits (or the equivalent) per semester in the student’s last two consecutive completed semesters
- Active participation in student activities or athletics
