When do I have to submit my immunization history and can I register and attend classes before I submit my immunizations?
NCC is now requiring that the immunization requirements be met prior to registration at the College.
Is a physical required for attendance at Nassau Community College?
Physicals are required for Nursing, Allied Health and to participate in a sport; otherwise no physical is required.
How do I obtain a copy of my immunization records?
Students with no records of past immunizations are advised to:
1. Check with your high school for immunization records
2. Check with your doctors past and present
3. Check with schools or colleges you have attended
4. Check immunization records from the Armed Forces, Health Department, etc.
What do I do if I can’t find any proof of immunization?
Students with medical Insurance can make arrangements with their doctors to get the required proof of immunity – either immunization or blood tests.
I don't have medical insurance and I need to get an MMR vaccine-what do I do?
Students without medical insurance must come in person to the Health Office after completing their Admissions Application. Referrals and advisement will be provided.
I’m transferring to another school, how do I get a copy of my immunizations?

Do I need my parents to sign anything if I am under 18 years of age?
Yes. If you are under 18 years of age, your parent must sign the Consent Form and have it notarized. This form is required in the event you need emergency medical care or medical treatment. After the form is notarized, it may be faxed, mailed or brought to Student Health Services, in person. You can find our contact information on the consent form.
Do I have to bring my immunization records in person?
No you don’t have to. You fax, mail or email your health information.
