Nursing

All Nursing students are required to submit a Health Form to the Office of Student Health Services, EVERY year. These forms must not be given to your Instructors. All first year Nursing students must complete the following requirements; A Physical Examination with a tuberculin Mantoux test done within 90 days of the first day of class. Those with a history of positive Mantoux must submit a copy of a recent (within two years) chest x-ray report. Diptheria/Tetanus immunization done within 10 years. Polio vaccine or any history is acceptable. Varicella, Measles, Mumps, and Rubella titers must be submitted with copies of the Laboratory Reports. Hepatitis B vaccine series must be started or a Declination Waiver must be signed. A urinalysis, hemoglobin or hemocrit must be done.

All Second Year Nursing Students with completed First year Health Forms are offered Physical Examinations free of charge. Spring student physicals are given from the first Tuesday in December until the last Thursday in January before the Spring semester begins, on Monday and Wednesday only from 8:30 a.m. to noon. Fall student physicals are given from the first Tuesday in May until the last Thursday in July, on Monday and Wednesday from 8:30 a.m. to noon. Some evening clinics are available, call 516-572-7767 for specific times.

Allied Health

All Allied Health students are required to submit a Health Form to the Office of Student Health Services, EVERY year. These forms must not be given to your Instructors. Mortuary Science

First year Mortuary Science students are required to complete the following: A Physical Examination, Tetanus/Diptheria within 10 years, Polio vaccine or any history is acceptable. A Tuberculin Mantoux Test done within 6 months of the physical exam, a Urinalysis and a Hemoglobin or Hematocrit must be done.

All second year students, with completed first year forms, must have a least two Hepatitis B vaccines prior to their embalming lab. You must submit proof of Hepatitis B vaccines to Student Health Services prior to your lab. A second year physical is NOT required.

Sports

All athletes are required to have a yearly physical which includes:

·Physical Examination

·Tuberculin skin test (Mantoux)

·Urinalysis

·First year sports must include eye & ear test

The student’s name must be on sports list provided by the coach, and be registered to receive a free physical at Student Health Services
